Abstract
Hashimotov tireoiditis (HT) najčešći je autoimuni tiroidni poremećaj koji je vodeći uzrok hipotireoze u područjima svijeta s dovoljnim unosom joda. Češći je kod žena nego kod muškaraca. Iako su identificirani mnogi genetski i okolišni čimbenici koji bi mogli potaknuti autoimuni odgovor, točni patogeni mehanizmi HT-a još uvijek nisu poznati. Odgovarajući unos joda, selena i željeza ključan je za zdravlje štitne žlijezde. Liječenje HT, koje je trenutno usmjereno na kliničke simptome bolesti te se liječi nadomjesnom terapijom levotiroksinom, trebalo bi u budućnosti djelovati na autoimuni mehanizam koji uzrokuje destrukciju parenhima štitnjače i posljedičnu hipotireozu. Bolje razumijevanje epigenetskih modifikacija i autoimunih patogenih mehanizama moglo bi pridonijeti točnijoj dijagnozi HT-a, adekvatnijem izboru pristupa liječenju i preciznijem predviđanju ishoda liječenja.
Abstract (english)
Hashimoto's thyroiditis (HT) is the most common autoimmune thyroid disorder that is the leading cause of hypothyroidism in areas of the world with sufficient iodine intake. It is more common in women than in men. Although many genetic and environmental factors have been identified that could enhance the autoimmune response, the exact pathogenic mechanisms of HT are still unknown. Adequate intake of iodine, selenium and iron is essential for thyroid health. The treatment of HT, which is currently focused on the clinical symptoms of the disease and is treated with levothyroxine replacement therapy, should in the future act on the autoimmune mechanism that causes the destruction of the thyroid parenchyma and consequent hypothyroidism. A better understanding of epigenetic modifications and autoimmune pathogenic mechanisms could contribute to a more accurate diagnosis of HT, an adequate choice of treatment approach and a more precise prediction of the treatment outcome.
